Enrollment flow

  1. 1. Enter member information and required consents in Google Forms
  2. 2. Receive guidance from the 3HAWK office for starting practice participation
  3. 3. Register for the communication tool provided to you, complete the credit card setup, and transfer the selected membership fee when applicable.

Required confirmation

Formal enrollment requires confirmation of the membership terms and privacy policy. Minors should complete the form only after receiving consent from a legal guardian.
